Working primarily in an ambulatory care setting, the Nurse Practitioner functions in an advanced practice role with a high degree of autonomy and clinical expertise. The NP is responsible for the comprehensive assessment, diagnosis, treatment planning, and longitudinal management of patients with complex neuro-functional disorders. As a key member of the interdisciplinary Neuro-Functional team, the NP supports patient-centered, evidence-informed care while contributing to program development, quality improvement initiatives, research activities, and education for patients, families, and healthcare professionals. Duties and Responsibilities: Utilizing advanced theoretical knowledge and expert clinical skills in neuro-functional care, the Nurse Practitioner will: Collect and interpret comprehensive health assessment data. Perform advanced assessments, establish medical diagnoses, and manage complex responses of individuals and families to actual or potential health problems. Develop, implement, and evaluate individualized treatment and management plans for patients with neuro-functional conditions. Facilitate continuity of care through consultation, follow-up, and coordination with interdisciplinary team members and community partners. Play a leadership role and perform quality improvement and administrative responsibilities. Participate in policy and procedure development and support program planning initiatives. Contribute to research activities and knowledge translation initiatives. Deliver, coordinate, and participate in education and professional development activities for patients, families, staff, and learners. Perform activities that further develop and strengthen the Nurse Practitioner profession. Act as a clinical resource and consultant to healthcare professionals, patients, and community agencies. Perform other duties consistent with the job classification, as required. Master's Degree in Nursing, Nurse Practitioner (Adult or Primary Care stream). Current Certificate of Competence with the College of Nurses of Ontario. Three to five years of nursing experience in an acute care or ambulatory setting with a neurology, neurosurgery, rehabilitation, or neuro-functional patient population. Demonstrated experience in advanced assessment, diagnosis, and management of patients with complex neurological or neuro-functional conditions. Canadian Nurses Association certification in Neuroscience Nursing is an asset. Experience in quality improvement, program development, and nursing research methodology. Eligible for or already holding a cross-appointment to an academic institution. Membership in the Registered Nurses' Association of Ontario (RNAO) and other organizations relevant to advanced practice nursing is an asset. As such, the NP will be uniquely positioned in guiding the development and evaluation of an innovative model of care for requiring Emergency Medicine Care.
