Patient Care Manager, General Medicine

About this team: We are a 28-bed general medicine unit dedicated to providing comprehensive care for patients with a wide range of acute and chronic medical conditions. Our patients range from individuals diagnosed with simple pneumonia to those with complex acute and chronic illnesses. This diverse patient population offers opportunities to acquire specialized knowledge in respiratory medicine, oncology and palliative care, pain management, geriatrics, and wound and skin care. Affiliated with various academic institutions, including the University of Toronto, our unit is committed to continuous learning and professional development, making it an ideal environment for nurses to grow and excel. n this role you will: • Establish and uphold unit goals that reflect the philosophy and standards of Sinai Health, while fostering an environment that supports staff development, clinical excellence, and high-quality patient care • Take a lead role in creating a culture of safety, risk mitigation, quality, and service at the unit level and across the portfolio, to improve practices and service delivery • Participate in the programmatic development of the clinical area • Manage unit-level budgets and lead resource utilization across the unit and broader program to ensure financial accountability and operational efficiency • Provide leadership in fostering and promoting a collaborative practice environment that is based on mutual respect among all team members • Lead, monitor and evaluate quality improvement and program development initiatives using data-informed methods that support departmental and hospital-wide priorities • Encourage staff participation in quality improvement activities for the unit and preparing quality improvement reports • Support the Hospital’s strong academic mandate for ongoing staff development and training of students from all healthcare disciplines • Recruit, develop and manage staff; complete regular employee performance appraisals and support the development of learning plans • Be accountable for the quality of care and patient safety at the unit level by actively maintaining a safe care environment. This includes risk identification, safety audits, staff accountability, and ensuring the appropriate structures, staffing, equipment, and procedures are in place • Establish, promote and maintain good public relations through effective communication within the organization • Foster effective labour relations through knowledgeable interpretation and implementation of collective agreements • Work with other members of the leadership team to support the implementation of evidence based practice and ensuring the dissemination of research findings to inform practice and improve patient outcomes • Respond as PCM on call as needed • Participate in the strategic planning process to drive organizational change and optimization of human resources programming and organizational development • Perform other duties consistent with the job classification as required Job Requirements: Mandatory • Current registration with a professional college under the Regulated Health Professions Act • Bachelor’s degree in a Health Sciences or related discipline • Master’s Degree in nursing, health or business administration, or health disciplines • 3 years of leadership experience in a health care environment • 2 years relevant clinical experience ◦ Applicants with proven equivalent recent and related training and experience may also be considered Preferred • Previous leadership experience in a unionized environment Skills and Knowledge • Knowledge of nursing and health care disciplines in clinical services and their scopes of practice • Established ability to facilitate evidenced based practice • Significant experience in quality improvement; success leading, monitoring and evaluating process/systems to identify and achieve efficiencies • Proficient in using Microsoft Office Suite; advanced understanding of the value of information technology and experience in the implementation and integration of strategies for systems and technologies • Excellent decision making, problem recognition, problem solving, communication, negotiation and conflict resolution skills • Flexible with the ability to work efficiently in a fast-paced, multitasking and dynamic environment • Experience in managing people and Human Resource processes, including formal performance reviews and engaging in staff development initiatives • Excellent interpersonal and communication skills; professional, diplomatic and confident with a proven ability to work with individuals across all levels of the organization and external stakeholders • Excellent organizational and time management skills with the proven ability to manage competing demands • Program planning and evaluation methodology skills are an asset • Demonstrated satisfactory work performance and attendance history If this sounds like you and you feel ready to build your career within health care, apply now and let us know why you’d be a great addition to our team.
