A role at the Reactivation Care Centre combines the close-knit collaboration of an interdisciplinary satellite team with the resources and support of our Mount Sinai Hospital Rehabilitation Department. In this setting, your physical therapy focus shifts from acute stabilization to evaluating and treating complex functional challenges in adults transitioning toward home or community placement. Working within a shared management structure and unified Electronic Medical Records (EMR) system allows you to collaborate closely with department peers and ensure seamless continuity of care as patient’s transition across clinical settings. Complex cases here often carry social, functional and financial factors that complicate the path home. You draw on creative problem-solving to work through equipment recommendations, disposition planning and the practical barriers between a patient and a safe discharge. Adaptability defines how you navigate the unpredictable nature of hospital care. Whether responding to unexpected patient surges, managing unplanned team absences, or pivoting when a scheduled patient is no longer medically appropriate to see, you re-prioritize your day seamlessly. About this team: The Reactivation Care Centre (RCC) offers therapeutic care to help patients rebuild strength, independence and confidence. This inpatient program supports recovery and helps patients get ready to safely transition from a hospital stay and return home or to another place in the community. It is designed for individuals who present with decreased mobility, independence and/or function as a result of an acute illness or surgery. Sinai Health operates the 16 bed inpatient unit with the UHN-RCC facility in partnership with University Health Network and Trillium Health Partners. We focus on patients with potential for functional improvement, providing tailored care plans and utilizing state-of-the-art tools to support their recovery journey. Depending on the reactivation goals patients typically experience an average length of stay of 30 to 60 days. In this role you will: • Assess, plan and provide therapeutic intervention for an assigned patient caseload according to physiotherapy scope of practice. • Compile appropriate documentation of physiotherapy activities • Discharge planning • Record workload measurement • Supervise Physiotherapy Assistants as per college guidelines • Participate in inter-professional clinics, rounds and discussions • Participate in teaching students from other disciplines • Mentor fellow colleagues, if appropriate • Perform other duties consistent with the job classification as required Job Requirements: Mandatory • Currently registered and in good standing with the College of Physiotherapists of Ontario • Strong experience in interdisciplinary education Preferred • 1 year of experience in the treatment of clients/patients • Membership in the Canadian Physiotherapy Association Skills and Knowledge • Excellent physiotherapy clinical skills with a broad variety of diagnostic groups including skills working with a geriatric population • Experience and skill in complex decision-making processes and in advancing best practice initiatives and exploration of innovative models of care • Experience in self-reflective learning and skill in ability to adapt and respond effectively to change • Excellent interpersonal and communication skills; professional, diplomatic and confident with a proven ability to work with individuals across all levels of the organization and external stakeholders • Demonstrated satisfactory work performance and attendance history If this sounds like you and you feel ready to build your physiotherapy career in the Reactivation Care Centre, apply now and let us know why you’d be a great addition to our team.
