**Licensed Practical Nurse, LPN - Hospice - PRN at VitalCaring Group in Lafayette, Louisiana, United States** **What you'll do** - Deliver Compassionate Hospice Care: Provide direct patient care in accordance with the Nurse Practice Act, implement individualized plans of care under the supervision of a Registered Nurse, deliver effective pain and symptom management, support patients with daily care needs while ensuring comfort, dignity, and safety - Coordinate & Communicate: Document care accurately and timely in the EMR, communicate changes in patient condition to the RN and care team, collaborate with interdisciplinary partners, participate in care planning and coordination to ensure holistic support - Commit to Excellence: Follow best practices and clinical standards in hospice care, maintain proper use of equipment and adhere to infection control protocols, participate in quality improvement initiatives and team development, continue professional growth through training and education **What they're looking for** - Active LPN/LVN license in the state of practice - Current CPR certification - 1–3+ years of nursing experience preferred (hospice, home health, or med-surg) - Strong clinical judgment, communication skills, and emotional intelligence - A genuine passion for patient-centered, end-of-life care - Valid driver's license, reliable transportation, and auto insurance **Details** - Location: Lafayette, Louisiana, United States - Type: Not specified - Salary: null
