The Opportunity 🚀 Carrot is expanding its telemedicine clinic with a network of part-time, fully remote physician consultants who deliver evidence-based virtual care to members across multiple U.S. states. Depending on your licensure, certification, and area of expertise, you will provide consultations in menopause and perimenopause care aligned to The Menopause Society and ACOG guidelines, early fertility support aligned to ACOG and ASRM guidelines, or both. Metabolic health is a core, cross-cutting component of this program. Weight, insulin resistance, and cardiometabolic risk are central to outcomes across every journey we support, from ovulatory function and preconception readiness to menopausal symptom burden and long-term cardiovascular and bone health. You should expect metabolic assessment and management to be a routine part of your visits, and clinicians comfortable prescribing and managing anti-obesity medications, including GLP-1 receptor agonists, within clinical protocols are especially encouraged to apply. This is a flexible, part-time contractor engagement of approximately 5-40 hours per week, with a target commitment of 10-15 hours per week. What You’ll Own 💼 In this role, you will: Provide virtual consultations (video and/or phone) within your area(s) of expertise, including assessment, diagnosis, treatment planning, and ongoing management of menopausal, fertility, and related hormonal and metabolic concerns. Apply current ACOG, ASRM, and The Menopause Society guidelines with individualized, shared decision-making. Assess and manage metabolic health as a routine part of visits, including prescribing and managing anti-obesity medications such as GLP-1 receptor agonists within clinical protocols where appropriate. Document visits accurately and on time, order and interpret relevant labs, and coordinate referrals to primary care, OB/GYN, endocrinology, behavioral health, or subspecialists as needed. Maintain active, unrestricted multi-state clinical licensure, complete credentialing and payer enrollment, and maintain eligibility for additional state licenses to expand coverage. Contribute to member experience and quality through strong virtual visit presence, effective use of telehealth and EHR systems, participation in chart audits and case reviews, and staying current with emerging research and regulatory changes. What Success Looks Like 📈 In your first 12–18 months, you will have: Established a consistent panel of high-quality virtual visits within your clinical area(s), with strong member experience and visit quality results. Maintained timely, accurate documentation and lab follow-up, with no outstanding credentialing or compliance gaps. Expanded your state licensure coverage in line with program needs, supporting broader member access. Integrated metabolic assessment and management, including GLP-1 protocols where clinically appropriate, as a routine part of your visits. Contributed to chart audits, case reviews, and clinical protocol refinement as the telemedicine program scales. About You ✨ You likely have: An MD or DO in Obstetrics/Gynecology, Reproductive Endocrinology and Infertility (REI), Internal Medicine, Family Medicine, or Endocrinology; or you are a Physician Assistant (PA) or Advanced Practice Registered Nurse/Nurse Practitioner (e.g., WHNP, adult/gerontology NP, FNP) with demonstrated expertise in the applicable clinical area. A minimum of 3-5+ years of active clinical experience in early fertility support, menopause care, or both, including evaluation, treatment, and longitudinal management. An active, unrestricted U.S. medical license; multi-state licensure is preferred and IMLC participation is strongly preferred. For clinicians practicing in the menopause specialty: MSCP certification from The Menopause Society, or certification actively in progress with a clear completion plan. Familiarity with ACOG, ASRM, and The Menopause Society guidelines. Prior telemedicine experience (preferred), with comfort managing visits entirely in a virtual environment and a strong virtual presence that supports shared decision-making over video and phone. Proficiency with telehealth technologies, including video platforms, secure messaging, electronic prescribing, and EHR/clinical documentation tools. Familiarity with HIPAA and applicable telemedicine privacy, security, and regulatory requirements, along with a HIPAA-compliant home workspace and reliable internet. Nice to Have ⭐ While not required, we would be especially excited if you bring: Clinical experience spanning both early fertility and menopause, enabling flexible coverage across member journeys. Experience working in a dedicated menopause clinic, fertility center, or comparable specialty program. Familiarity with early fertility workup, including AMH testing, ovulation induction, and preconception optimization, with appropriate escalation to in-person ART evaluation. Experience prescribing and managing anti-obesity medications, including GLP-1 receptor agonists (titration, side-effect management, monitoring, and discontinuation planning); obesity medicine training or ABOM certification is a significant advantage. Prior experience in digital health, health benefits, or multi-state telemedicine environments. Experience contributing to clinical protocol development, quality improvement, or member-facing clinical content. Experience supporting diverse and historically underserved populations, including LGBTQ+ and gender-diverse individuals. Fluency in an additional language.
